POLICY NOTICE- EFFECTIVE SEPTEMBER 1ST 2016
15 MINUTE APPOINTMENT-In order to better serve our clients, the time allotted for a routine office visit or routine adjustment will be limited to a 15 minute time slot. If you choose to ask the doctor to spend more time with you than the allotted amount of 15 minutes, and if the time is available, then each additional 5 minutes will be charged $10.00. Otherwise, we ask that you make an another appointment to deal with any additional needs. This is to minimize wait times for all of our clients and to ensure fairness of services and charges. This will improve our patient flow and timeliness.
LATE POLICY-In order to prevent your time with the doctor from being cut short, it is very important for you to be on time for your appointment. If you are late for your appointment, the minutes that you are late will be forfeited from your allotted time with the doctor. Any time spent after the scheduled and allotted time will be charged $10.00 for every additional 5 minutes. To avoid additional fees, we ask that patients show up 15 minutes before their appointment.
CANCELLATION AND NO-SHOW POLICY- A 24 hour notice is requested for any cancellation of appointments. We reserve your appointment time especially for you. If you are unable to make your appointment we require a phone call or voice mail before the start of the business day to notify us of your inability to make the scheduled time. If an appointment is missed and we have not received notice then a $40.00 dollar charge may occur
PAYMENT POLICY- Payment is due at time of service. For your convenience, we accept money orders, cash, local checks, Visa, Mastercard, American Express, and Discover. Any other arrangements need to be approved before service is rendered. As always, our initial consults are free. The doctor will notify you when the consult has concluded and before any procedure that will incur cost.
